By Wednesday evening eight people were suddenly dead, although speculation had pumped this figure to nearer one hundred. To every death there was a common theme: the afflicted were all young, or at least in early middle-age. Soon there was a growing crowd outside Surat's New Civil Hospital. Wards were close to capacity, the wail of ambulance sirens was audible across town. In the main lobby a huge concrete concourse, with rusty splashes of betel phlegm along the walls—crowds were fighting to get to the drug counters. From the balcony above, a line of doctors scanned the melee: dressed head to toe in white, their faces masked, only their dark eyes visible. "As if the deaths were not bad enough," said Dr. Shailendra Vajpeyee, "no one had any idea of the cause." An emergency meeting was called. Senior doctors, surgeons and administrators argued furiously. One said it was dengue fever, another a mutant strain of malaria. A third carried news from a nearby hospital, where two more people had died, both with the same symptoms. Finally, into a room suddenly silent, one of the doctors spoke softly, with certainty. "It's the plague."

Competition, recognitiion and disease

The book really warrants 3 1/2 stars, but I am rounding up. Marriott's _Plague_ is as much about bubonic plague as a disease as it is the competition between Alexandre Yersin and Kitasato Shibasaburo to discover the bacillus itself, its vector and a vaccine. In many respects it is reminiscent of Flu in its discussion of the disease and the race between scientists to understand it. Like Kolata's book, the diffference between competing immunologists in temperment and technology as well as their relations with the press could not be more different, making for dramatic reading. Marriott connects the scientific competition of the late 19th century to events in the late 20th century by relating contemporary stories of the plague and the similarity in reaction by physicians, governments and the populous at large.

The real heart of the story is the race to discover the microorganism that is bubonic plague. In 1894 Hong Kong, the plague broke out and spread with typical virulence among the poor and crowded parts of the city. Two bacteriologists were dispatched to solve the problem of its spread: Kitasato Shibasaburo from Tokyo, Yersin from Vietnam (then a French colony.) In Hong Kong, the British consulate intentionally blocked Yersin at every turn, heightening the real-life drama as each sought to solve the mystery of this disease. Matters became complicated as Shibasaburo rushed to publish in order to gain the recognition and fame he so desired. As a result of official favor, hasty publication and even active attempts at later tempering of inaccurate scientific conclusions (the result of the rush to publish), it would be years before proper credit of the discovery of the bacteria was granted to Yersin. Marriott's retelling of the competition and the reluctance of the scientific community to recognize Yersin's work makes for compelling reading.

There is an uneveness in his story telling, however. Marriott periodically oscilates between 1894 Hong Kong and 1994 Surat, India when there were fears of an outbreak of plague. The connection between the two is a bit tenuous (bubonic plague will spread to India, Africa and North America as a result of the 1894 outbreak in Hong Kong via shipping lanes), and tends to distract from the larger story of Yersin's work. That Marriott was showing how dangerous the plague remains, how the medical and governmental institutions respond (poorly) to outbreaks of the plague, and the debt owed to the work of Yersin (and Paul-Louis Simond, a pioneer in developing a vaccine) makes sense, but I think it ultimately distracts from the main story.

Unlike other books on the plague (The Black Death and the Transformation of the West (European history series), Black Death, Black Death), the focus on its place in a contemporary setting is a strength of the book, I think. I also liked the immunological approach to the subject, similar in feel and organization to The Great Influenza: The Story of the Deadliest Pandemic in History or The Ghost Map.. Recommended.
